| Quick Answer |
| India has five prescription routes worth knowing in 2026. Tirzepatide (Mounjaro, Yurpeak) and semaglutide injection (Wegovy) are approved for chronic weight management. Ozempic and Rybelsus contain semaglutide but are approved only for type 2 diabetes. Generic semaglutide arrived after the patent expired on 20 March 2026, and orlistat remains the older capsule option. Every one of them needs a doctor’s prescription. |
How Weight Loss Medicines Are Regulated in India (CDSCO)
Every weight loss medicine in India passes through one gate: the Central Drugs Standard Control Organisation (CDSCO), headed by the Drugs Controller General of India. CDSCO decides what a drug may be sold for, and that approved use matters more than the molecule printed on the box.
Semaglutide is the clearest example. The same molecule sits inside Wegovy, Ozempic, and Rybelsus, yet only Wegovy carries a weight management approval in India.
2026 tightened the rules in practice.
On 10 March 2026, CDSCO issued an advisory telling manufacturers that disease “awareness” campaigns can count as surrogate advertising of prescription medicines. Later that month it asked state drug controllers to watch online pharmacies, wholesalers, and wellness clinics, and inspectors audited 49 businesses across the country.
So a legitimate weight loss injection in India comes with a prescription from a registered medical practitioner, a pharmacy bill, and an unbroken cold chain. A pen sold through a reel with a discount code does not.
Available Medications
Two families cover the GLP-1 drugs available in India plus one older option. GLP-1 medicines copy a gut hormone that tells the brain you have eaten enough and slows how fast the stomach empties. Orlistat works inside the gut instead, blocking part of the fat in a meal from being absorbed.
Tirzepatide (Mounjaro)
Eli Lilly launched Mounjaro in India in March 2025, first as vials and from August 2025 as a once-weekly KwikPen. Cipla sells the same Lilly-made medicine as Yurpeak. Tirzepatide acts on two gut-hormone receptors, GIP and GLP-1, which is what separates it from semaglutide.
Its Indian approval covers type 2 diabetes and chronic weight management in adults. In SURMOUNT-1, a 72-week trial in 2,539 adults without diabetes, weight loss rose with dose, and people on the highest dose lost up to 20% of their body weight.
The dose climbs every four weeks, and nausea is common on the way up. That climb is exactly why it belongs under a doctor’s supervision, with someone adjusting the pace when your stomach protests.
Semaglutide injection (Wegovy/Ozempic)
Novo Nordisk launched Wegovy in India in June 2025 as a weekly FlexTouch pen in five strengths, from 0.25 mg to 2.4 mg. It is approved for chronic weight management, and CDSCO added an indication in July 2026 for a form of fatty liver disease (MASH) with liver scarring.
In STEP 1, 1,961 adults without diabetes lost an average of 14.9% of body weight over 68 weeks on 2.4 mg, against 2.4% on placebo.
Ozempic arrived in December 2025, but India has approved it only for type 2 diabetes, at doses up to 1 mg. Using it for weight loss is off-label and should only be decided by your doctor.
Oral semaglutide (Rybelsus)
Rybelsus has been sold in India since January 2022 as a daily tablet in 3, 7, and 14 mg strengths. It is approved here for type 2 diabetes only, not for weight management.
The weight effect at these doses is modest. In PIONEER 1, adults with type 2 diabetes on 14 mg lost 2.3 kg more than placebo over 26 weeks.
A higher-dose oral semaglutide approved for obesity in the US and the EU is not approved in India as of October 2026. Generic oral versions from Torrent (Semalix) and Dr Reddy’s launched in 2026, again for diabetes, so a tablet is a diabetes decision your doctor makes rather than a shortcut to a weight loss injection.
Generic semaglutide
The Indian patent on semaglutide expired on 20 March 2026, and generic semaglutide in India arrived within days. Natco, Sun Pharma, Dr. Reddy’s, Zydus, Torrent, Alkem, and Glenmark were among the first wave, in vials and pens.
Here is the detail most comparison pages skip. The approved use differs by brand. Sun Pharma sells Noveltreat for weight management and Sematrinity for diabetes, while Dr. Reddy’s Obeda and Glenmark’s Glipiq launched for type 2 diabetes.
Same molecule, different label.
Vials allow finer dose steps but require you to draw up each dose yourself, which carries a real risk of error without training. Read the indication on the carton and let the prescribing doctor choose the format, because the cheaper pack is not automatically the equivalent one.
Orlistat
Orlistat is the veteran here: a capsule taken three times a day with meals that contain fat. It blocks the enzymes that digest fat, so a share of that fat leaves the body unabsorbed.
In the four-year XENDOS trial, adults with obesity lost 5.8 kg on orlistat against 3.0 kg on placebo, and new cases of type 2 diabetes fell from 9.0% to 6.2%.
Indian kitchens make orlistat tricky. Ghee and cooking oils sit almost entirely in the fat column of the Indian Food Composition Tables 2017, so a tadka-heavy dal or a plate of puri can trigger its side effects quickly. The Xenical prescribing information notes that gut effects rise when more than 30% of daily calories come from fat, the same ceiling the Dietary Guidelines for Indians set for total fat. A doctor and a registered dietitian should shape how it fits your meals.
Comparison Table (form, average loss, approx. price)
Here is how Mounjaro, Wegovy, and Rybelsus compare in India, alongside the other options. Trial averages describe groups of people in specific studies, not what any one person will lose. Prices are maximum retail prices (MRP) reported between August and October 2026 and change often.
| Medicine | Form | Approved in India for | Average loss in trials | Approx. price per month (MRP) |
|---|---|---|---|---|
| Tirzepatide (Mounjaro, Yurpeak) | Weekly injection (vial or KwikPen) | Weight management, type 2 diabetes | Up to 20% of body weight at the highest dose (SURMOUNT-1, 72 weeks) | ₹13,000 to ₹26,000 |
| Semaglutide injection (Wegovy) | Weekly pen | Weight management, heart risk reduction, MASH | 14.9% (STEP 1, 68 weeks) | ₹5,660 to ₹16,400 |
| Semaglutide injection (Ozempic) | Weekly pen | Type 2 diabetes only | Not trialed as an obesity treatment at Indian doses | ₹5,660 to ₹9,100 |
| Oral semaglutide (Rybelsus) | Daily tablet | Type 2 diabetes only | 2.3 kg more than placebo at 14 mg (PIONEER 1, 26 weeks, type 2 diabetes) | ₹8,900 to ₹10,900 |
| Generic semaglutide | Weekly vial or pen; some daily tablets | Varies by brand: diabetes or weight management | Same molecule as the originals; check each brand’s approved use | ₹1,290 to ₹8,000 (injectables) |
| Orlistat | Capsule, three times daily | Weight management | 5.8 kg vs 3.0 kg on placebo (XENDOS, 4 years) | ₹2,500 to ₹6,800 |
Who Is Eligible
The Indian labels for Wegovy and Mounjaro follow the global trial design: adults with a BMI of 30 or more, or 27 or more with a weight-related condition such as type 2 diabetes, high blood pressure, high cholesterol, or sleep apnea.
Indian clinical thinking draws the line much lower. The 2009 consensus called a BMI of 23 overweight and 25 obese, and the 2025 revised definition of obesity in Asian Indians treats a BMI above 23 as obesity, with a more serious second stage when excess waist (90 cm or more in men, 80 cm or more in women) or a raised waist-to-height ratio joins in.
The scale of the problem is not small. The ICMR-INDIAB study estimated 254 million Indian adults with generalised obesity and 351 million with abdominal obesity.
Picture a 40-year-old in Pune who is 1.65 m tall, weighs 72 kg and has a 94 cm waist. Her BMI of 26.4 counts as obesity by Indian criteria, yet she sits below the label line. Whether medicine has a place for her is a judgement for a doctor who knows her full history, not a gap to close with an online order.
Some people should not take these medicines at all. The Wegovy prescribing information rules out anyone with a personal or family history of medullary thyroid cancer or MEN 2, and GLP-1 medicines are stopped in pregnancy. Orlistat is unsuitable for people with chronic malabsorption or cholestasis.
Safety & Side Effects
Stomach trouble is the price of entry for GLP-1 medicines. In STEP 1, 74.2% of people on semaglutide reported gut symptoms such as nausea, diarrhea or constipation, against 47.9% on placebo, and most cases were mild to moderate and eased with time.
Some stop anyway. Side effects led 7.0% of people on semaglutide to quit in STEP 1, and between 4.3% and 7.1% on tirzepatide in SURMOUNT-1.
Rarer risks deserve a clear mention. The Wegovy prescribing information warns about pancreatitis, gallbladder problems, and kidney injury linked to dehydration, which is why a doctor checks your history before the first dose and stays reachable afterward.
Eating less also makes protein harder to hit on a dal, roti, and sabzi plate, and muscle can slip away with fat. A registered dietitian can help with that.
Orlistat’s side effects are different and very visible: oily spotting, urgency, and loose, fatty stools, all worse after rich meals. Because it also blocks fat-soluble vitamins A, D, E and K, the Xenical label advises a daily multivitamin taken at a separate time.
| Conclusion |
| India’s prescription options for weight have widened fast: two approved weight management injections, a wave of generic semaglutide, diabetes-only versions of the same molecule, and orlistat. The approved use on the label, your own health history and a doctor’s supervision should decide which, if any, fits you. |
FAQs
1. Which weight loss medicines are approved in India?
As of 2026, Wegovy (semaglutide) and Mounjaro or Yurpeak (tirzepatide) are approved for chronic weight management, along with orlistat and generic semaglutide brands that carry a weight management indication, such as Noveltreat. Ozempic and Rybelsus contain semaglutide but are approved in India only for type 2 diabetes.
2. Is generic semaglutide available?
Yes. Generic semaglutide has been sold in India since the patent expired on 20 March 2026, from companies including Sun Pharma, Dr. Reddy’s, Zydus, Natco, Torrent, and Alkem. Each brand has its own approved use, diabetes or weight management, so check the label and buy only with a prescription.
3. Are there oral (tablet) options?
Orlistat capsules are the oral option approved for weight management in India. Rybelsus and the generic oral semaglutide tablets from Torrent and Dr Reddy’s are approved for type 2 diabetes only. A higher-dose semaglutide tablet for obesity is approved in the US and the EU but not in India as of October 2026.
4. Who can be prescribed them?
Indian labels for Wegovy and Mounjaro cover adults with a BMI of 30 or more or 27 or more with a weight-related condition. Indian criteria define obesity from a BMI above 23, so a registered medical practitioner weighs your full health picture before deciding. Pregnancy and some thyroid histories rule these medicines out.
